The variation between a surgical nose job (surgical rhinoplasty) and a non-surgical nose job (liquid rhinoplasty) Is groundbreaking. Here we are going to discuss the differences between these two options. The chances are, there has been a lot of money spent and a lot of time hiding behind closed doors whilst the bruising and swelling reduce after they have experienced the surgical nose job procedure. We’re going to break down the process and information relevant to this cosmetic procedure in order to highlight the intensity and the somewhat groveling experience involved and compare this to the simplicity, the financial saving, and the minimal downtime or pain involved with the non-surgical liquid rhinoplasty.

what surgical rhinoplasty involves.

First of all; it’s important to note that generally speaking, nose reshaping when done surgically is carried out under a general aesthetic. From this alone, we know that admission to a hospital or private clinic for several pre-operation assessments is necessary, and after any general aesthetic: medical supervision is paramount during the recovery. You can expect to be in surgery from 1.5-3 hours, and then even still, once the worst part is over it can be months before you actually see the true healed results of the procedure. Let’s face it; it’s 2022 – nobody’s got time for that!!

Did we mention it will also cost you anything from £5-8000!!

Depending on the type of surgery that is desired, i.e.- reduction, augmentation, nostril reshaping, etc. the surgeon may carry out a variety of different techniques to achieve this. Some of these techniques include:

  • removing the cartilage and bone from the nose
  • taking cartilage from the ears
  • taking bone from the hips, elbow, or skull
  • rebuilding the nose using various other forms of bone or cartilage from the body (medically known as grafting)
  • nostril reduction/ reshaping by breaking the nose bone and rearranging the cartilage

…these are but a few to name the gruesome steps involved with reshaping and resizing the nose.

Our solution is liquid rhinoplasty. This is a non-surgical procedure that is performed using a small gauge needle that injects a varied amount of hyaluronic acid into the area of the nose in which you are wanting to reshape- it’s as simple as that!

Areas that can be treated include the dorsal hump and the nasal tip.

The product that is used (hyaluronic acid) is the same product that is commonly used for Dermal fillers in other areas of the face such as the cheek, the chin, the jaw, and the lips.

Various different brands and types of hyaluronic acid can be used for this treatment-  a few of the industry favorites include Juvéderm and Eloquence. Over time and throughout medical and cosmetic studies, hyaluronic acid has gained a phenomenal reputation for being an effective and safe alternative to surgery.

A huge plus side to undergoing the liquid rhinoplasty treatment is that a general anesthetic is not necessary. When having this procedure done, the patient is awake and conscious throughout. Although it sounds scary, the product used usually contains lidocaine anyway. So the procedure is never too uncomfortable and usually starts to numb. Liquid rhinoplasty usually takes around 15 minutes (which of course is no comparison to the three-hour surgery if you were to undergo full surgical rhinoplasty).

Because you are awake during this treatment, you will often be handed a mirror by your practitioner which means that you can even watch the treatment being done – giving you the reassurance that you have more control and awareness of what’s being done!

There is no downtime or recovery time for this procedure, which means that as soon as your appointment is finished you are free to go and carry on with the rest of your day. The best part about it-the results of the procedure is immediate.

Another pro for liquid rhinoplasty is that the treatment is temporary. Therefore, if you are unhappy with the results, they are completely reversible. The treatment and product can either be immediately dissolved using Hyaluronidase, or you can wait several months as the product will naturally wear off. This treatment usually costs between £150-500 (depending on the practitioner’s costs and pricing)Which also means it’s completely affordable should you want to repeat the treatment every 8 to 12 months.

However, we recommend receiving the Biotin vitamin via either intramuscular injection or IVNT drip. Although this way of taking the vitamin can cost slightly more, the upside is first that you do not have the responsibility of remembering to take a tablet every day as it is administered as a booster once a month, but secondly, the intramuscular injection or intravenous drip allows the body to absorb up to 90% of the concentrated vitamin-this giving you far more benefits and efficiency in the absorption of all that this miraculous beauty-boosting vitamin has to offer!

The most dangerous element of a dermal filler injection is the possibility of injecting into an artery. The tip of the needle is the single most important component to be aware of in terms of position when doing filler injections via either a cannula or a needle. The type of filler you use is not relevant. However, this is based on hyaluronic acid based dermal fillers. BDDE cross linked dermal fillers. They can be from any brand whether it is Juvederm ultra or Vycross. Teosyal or Teoxane. Belotero or Revolax. The brand does not matter. This can happen with any HA based filler. It can lead to necrosis of the tissue and this can leave scarring or even blindness.

Cannula is considered to be safer than needle. However, if a cannula cannulates an artery it can often be more devastating than a needle injury. There are many ways in which you can try to avoid a vascular occlusion. But the single most important thing is to have a comprehensive understanding of an injectors anatomy. We cover this in great detail on our Dermal Fillers Foundation Training course.

There are multiple signs a vascular occlusion may have occurred.

  • Livedo Reticularis
  • Slow capillary refill

There are more but these two are the most significant.

Livedo Reticularis can present itself in the form of mottling. And slow capillary refill can be seen by pressing an area of skin and seeing how long the blood takes to return. If the capillary refill is slow. More than 4-5 seconds. It maybe necessary to seek specialist attention.

If it is determined that VO is definately present. An emergency protocol should be applied. This can include several items

  • Massage
  • Heat pack
  • Viagra
  • Aspirin
  • Hyalase – Hyaluronidase (Or equivalent – Disolvidase for example)

In a specific order these can be utilised in order to resolve a vascular occlusion. Dissolving should be the last possible option but it should always be available. In many cases a single vial of hyalase is not enough. One vial is 1500iu. Often many are needed. So we recommend always stocking at least 10. Preferably 20 and all of the appropriate needles and syringes and anything else that maybe required.

You should have a well documented protocol that allows you to fully understand what your steps should be in any given emergency situation. See our diagram below for a comprehensive emergency protocol.

Hyalase Emergency protocol for dermal fillers

If you have an ultrasound machine this can also be used in order to do vascular mapping of the face in order to be able to build up an accurate understanding of exactly what is going on under the skin and exactly where the vascular occlusion is. Once found it can be easily injected with hyalase and dissolved. We offer specialist ultrasound training with a very high level HD 20Mhz ultrasound scanning device. This ultrasound training for injectors is a very specialist training course and should be for highly experienced aesthetic practitioners. The ability to check depths of arteries and veins and muscles and even facias via ultrasound can be truly revolutionary in regards safe injecting and superior vascular occlusion emergency response.

Even with non BDDE cross-linked products such as profhilo. Sunekos. Jalupro. VO is still possible. If the pressure is significant enough and in the wrong area. And too much product injected. It’s often considered safe. But in the wrong hands it is not safe. Again for these HA based products. Hyalase and your emergency protocol would be important to have to hand.

Botox and other botulinum toxin products such as boccouture and azzalure do not cause vascular occlusion and dissolving of these products is both unnecessary and also impossible. Even with this knowledge it’s very important to have the correct Anti wrinkle injections and botulinum toxin training to make sure the procedure is done correctly and most importantly safely. Can PDO threads cause a VO? Or vascular occlusion? PDO threads can also not be dissolved. However if inserted incorrectly could cause damage to blood vessels and again its extremely important that they are administered correctly and safely. What is Carboxy Therapy?

Microsclerotherapy

What is Microsclerotherapy?

Micro-sclerotherapy is the injection of a sterile chemical into blood vessels, using small needles, causing inflammation and irritation leading to scarring of the inner lining of the vessel. This then causes the vessel to shrink, collapse, harden or close up and eventually fade away which resolves the cosmetic problem. Several sessions may be required to obtain maximum results.

This treatment is used on thread veins on the legs or body, provided they are beneath the heart. Microsclerotherapy is one of the most effective and well-established treatments for spider veins or telangiectasia. It is extremely effective on spider veins on the legs, and involves the injection of a solution, or ‘sclerosant’, with a very fine needle into the problem blood vessel.

There is little to no recovery time with this treatment, patients don’t normally find the injections painful.

Celluerase

What is Celluerase?

Celluerase is a revolutionary Industry to reduce the appearance of cellulite. Fibrous connective tissue pulls through the subcutaneous layer causing the ‘fat’ cells to push through and create an orange peel effect. Special blades are used under the skin to ‘cut’ the fibrils and releasing the pull thus minimizing the appearance of cellulite. This can be performed on the legs and buttocks.

This is a brand-new treatment In the UK and is gaining popularity at a vast rate. The results are INSTANT and once treated should not need to be treated again. Bum filler

What is Bum filler?

This treatment course will cover the complete remodelling of a person’s buttock area as well as contouring hip dips. Special contour filler, which comes in 10ml syringes is used with a 100mm canula to shape and volumize the buttocks. Lidocaine is injected prior to the filler to ensure client comfort.

Over recent years the number of breast augmentations have dropped by 20%, according to the British Association of Aesthetic Plastic Surgeons. Meanwhile, buttock enhancements have been rising by as much as 15% a year since 2013. As celebrities like Kim Kardashian promote a love for bigger bottoms, this obsession is not abating.
